In consideration of my child being allowed to participate in ENC Elite activities, I hereby release, waive, discharge, and hold harmless ENC Elite Soccer Club, its directors, coaches, trainers, volunteers, employees, affiliates, sponsors, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur during participation. I understand it is my responsibility to inform ENC Elite of any medical conditions, allergies, injuries, or restrictions that may affect participation. I also grant permission for ENC Elite to use photographs or videos of my child taken during club activities for promotional, marketing, and social media purposes without compensation.
